Frailty

The surgery offers frailty assessments for patient's recently discharged from hospital or are aged 50 years and over.  The assessment can involve the patient's family member's, next of kin, carer to identify if support is also needed for the patient's family/ carer.

The assessments allow the health clinician to identify any unmet needs of care and take steps to put a care package in place.

Care plans can be put in place for short or long term needs of care.

Changes to care plans can be discussed with your GP or a health clinician.

The surgery also offers carer's health checks annually.

If you or your family would like further advice or information about having a frailty assessment please contact the surgery on telephone number 01772 323 021 to book a telephone or face to face appointment.
